Who the site is for

This site is intended for U.S. medical professionals, healthcare facilities, suppliers, distributors and purchasing organizations. Product information is provided for professional use and is not a substitute for the product’s instructions for use or for clinical judgment.

Accounts

• Accounts are for businesses. Lavior reviews every application and may approve, decline or close an account at its discretion.
• You agree that the information you provide is accurate and that you will keep it up to date.
• You are responsible for keeping your login private and for activity under your account. Tell us right away at support@lavior.com if you think it has been used without permission.

Pricing and orders

• Account pricing is shown to approved accounts after login. Prices may change; the price that applies is the one on your invoice.
• An order is accepted when we send its invoice. We may decline or cancel an order, for example when stock is not available.
• Shipping is added to the invoice. Orders ship once the invoice is paid.
• Questions about an order, a shipment or a damaged delivery: contact support@lavior.com.

Product information and coding

Use each product only as described in its labeling and instructions for use. Coding and coverage information on this site is general reference information, not legal, billing, coding or reimbursement advice, and a HCPCS code does not guarantee coverage or payment. Lavior is not a billing company and does not submit claims.

Acceptable use

Do not misuse the site: no attempts to access other accounts or non-public areas, interfere with the site’s operation, scrape it in bulk, or submit false or unlawful information. Do not send patient-identifiable information through the site.

Intellectual property

The site’s content, including text, images, the Lavior™ name and AGS-RIED™, is owned by or licensed to Lavior Pharma, Inc. You may use it to evaluate and order our products, and not for other commercial purposes without our written permission.

Links to publishers, government sites and partners are provided for convenience. We are not responsible for their content.

Disclaimers and limitation of liability

The site and its content are provided “as is”. To the extent the law allows, Lavior disclaims implied warranties about the site and is not liable for indirect, incidental or consequential damages arising from use of the site. This does not limit any rights you have under the law that cannot be limited, or any written agreement or product warranty that applies to your purchase.
